How to win a visa appeal?

One of the problems these days is that visa officers in embassies are stiff in issuing visas. They reject visa applications generally and grant visas only in exceptional cases. This happens mostly in cases where the visa application is from third world countries like India, Bangladesh, Srilanka, Philipines, Iran, Afghanistan, Nepal, Bhutan, Pakistan, Zambia, Kenya, Sudan, Egypt and so on.
